Use the same phone number to confirm,
cancel, or reschedule an appointment.
2. What should I do if I have a question for my doctor or
nurse?
Ø
Call 212-305-6354, choose
your language option, then press (4) to speak with our nurse.
Ø
If the phone line is busy, leave a message
and our staff will call you back the same day. Please speak clearly,
include your full name, date of birth, doctor’s name, telephone number,
and the reason for calling. For refill request please leave us the exact
medication name and pharmacy phone number.
3. What if I have a question and the office is closed?
Ø
A doctor is always on-call to answer
questions after the office is closed. You can reach them by calling the clinic
number at 212-305-6354, and
pressing (1) then (2) to
speak to the answering service who will take your information and contact the
doctor-on-call.
4. How can I get refills of my medications if I run out?
Ø
Tell your pharmacy to call our office at
212-305-6354 and ask for refills of the medications you need.
Ø
We may not be able to fill prescriptions
over the phone if you have not been seen by a doctor in the last six months or
have missed appointments.
Ø
If you cannot get an appointment with your
doctor and you have not been seen for 6 months, you can come to our Walk-in
Clinic in Room 214 to speak with a nurse about your refills.
5. What do I do if I have an urgent medical problem and need
to see a doctor on the same day?
Ø
We have a Walk-in

Monday - Friday.
Ø
You will be seen by another doctor and
he/she will take care of your medical problems and communicate with your
primary doctor.
6. How can I leave forms for my doctor to fill out?
Ø
You can come by and leave forms with our
clinic staff any day.
Ø
Please make sure you have
filled out your part of the form (name, address, confidential release
signature).
Ø
Give your doctor 1-2 weeks to complete the
forms.
Ø
If you would like us to mail you the forms
once they are filled out, please leave your address with the clinic staff with
your forms and we will mail them to you.
7. If my doctor ordered lab tests for me, how do I get them
done?
Ø
You do not need a laboratory form to get
your lab tests done in our clinic system. You can bring your medical record
number (on a sticker from the front desk of your clinic room) or the discharge
paper from the front desk that says you should go to the lab.
Ø
If your doctor has told that you need lab
tests, just come in to the lab on the first floor and the lab orders will be in
the computer.
Ø
Check to see if your doctor wants your
blood tests done first thing in the morning before you have eaten.

Lab hours: 8:30 am - 4:30 pm, Monday -
Friday.
