The Ralph Lauren Center for Cancer Care & Prevention (RLC) is a community based Center located in Harlem designed for the prevention, diagnosis, and treatment of Cancer. In addition to cancer treatment services, RLC also provides screening for breast, cervical, colon and prostate cancers. Screening colonoscopies are performed in our new state of the art Endoscopy Suite which was opened on January 28, 2008.

Referral Procedure

To make an appointment (patients can call directly):

I. Call 212 987-1777 Monday to Friday (9 am to 5pm).

II. Please provide the following information:

· Patient’s name, D.O.B, S.S#

· Patient’s address, telephone #, alternate tel #

· Insurance information

· Referring MD name & contact number

III. Screening and prep instructions will be provided at the RLC.

IV. The referring practitioner will receive an original report of findings and the patient will be referred back for all other medical issues.

Insurances accepted:

We accept Medicare, Medicaid, and most health insurance and managed care plans such as; NY Presbyterian, Affinity, Aetna, Amerigroup, Centercare, Cigna, Elderplan, Empire BC/BS, Empire Medicare, GHI, Health First Medicare and Medicaid, Healthnet, HIP, Magnacare, Multiplan, Oxford Health Plan, United Healthcare, 1199. We currently have grant funding available to provide colonoscopies at no cost to uninsured patients.
