Pre-Procedure Check List

1. What is your procedure certification status, who is your supervisor?
2. Did you identify your patient by 2 unique identifiers, and obtain an informed consent.  Make sure to clearly review the procedure step by step.  Does your patient know what to expect? 
3. Review in your mind, all the steps involved. Do you have all of the necessary equipments (including specimen bottles and labels)?
4. Ensure all equipments are properly opened, and kept sterile. 
5. Are you comfortable?  Check lighting. Check bed position. Can you access everything you need.
6. Is the patient comfortable and ready to proceed?
