Providers requesting Jobst stockings can write the prescription and send to pt. to the Stahl Surgical Rep. (on VC 10 Tues. and Fri., 10 AM-11 AM)

Outside referrals must bring pertinent radiology studies (films), labs and pathology slides

Referring provider should order appropriate studies (e.g.: mammogram, abd. Ultrasound, NIFS) to be completed prior to the appt.

Patients requiring evaluation for Obesity surgery are to be referred to the NYPH Obesity Center (x54000)

Patients requesting cosmetic Plastic Surgery are to be referred to NY Weill Cornell Plastic Surgery Clinic

Patients requiring reconstructive Plastic Surgery consultation (including hidradenitis suppurativa) are to be referred to the Dept. of Plastic Surgery (x5868)
For patients requiring Thoracic Surgery consultation, referring provider must page the Thoracic Surgery House Officer (PGY III)
Patients requiring Thyroid Surgery consultation are to be referred to Thyroid Clinic (x62645)
Patients with bony growths (e.g.: ganglion cyst) are to be referred to Ortho Clinic (x55187)

Surgery Service hospital discharges are to have their post-discharge follow-up appointment made in the clinic of the Attending Surgeon of record unless otherwise ordered:

Dr. Amory: Wed. PM, Allen 3FE

Dr. Allendorf, Dr. Bessler, Dr. Chabot, Dr. Davis, Dr. Schrope: Thurs. AM, VC 10

OVER-BOOKINGS can only be approved by a Surgical Chief Resident or Attending Physician.

Referring physicians requesting over-booking for an urgent appointment must obtain approval from a Surgery Service Chief Resident or Attending Physician. Referring physician may call the Consult Resident (Colo-rectal and Upper GI-b 8882, General Surgery-b 8883) or a Surgical Attending.

Enter the name of the approving physician (as well as the referring physician) when scheduling the appointment.
